COVID On-Site Service Agreement
In view of the evolving COVID-19 pandemic, Holco Metrology Solutions requires commitment of the following terms for all on-site visits. The health and safety of our clients, employees, families and communities are our top priority and we are committed to doing our part to prevent the spread of the virus to you and others. This form must be received prior to scheduling service.
HMS Technician(s) Will:
- Monitor personal health daily and notify Host immediately if showing symptoms of fever, cough, chills, loss of smell or taste, etc.
- Observe mandatory and/or recommended state, local and company-specific COVID guidelines and/or protocols at all times
- Properly wear face covering and observe social physical distancing while on-site
- Confine their movements only to those necessary to perform the required services
- Provide all service documentation electronically upon completion of service
- Thoroughly disinfect the machine and any touched surface upon completion of service
Host Will:
- Ensure employees properly wear face coverings at all times in areas where our Technician(s) will be working or passing through. Face coverings must cover both nose and mouth, and are not a substitute for – but an addition to – maintaining appropriate physical social distance.
- Provide for generous Social Distancing between their employees and our Technician(s)
- Limit # of people around technician as close to zero as possible.
- Provide physical plant entrance for our Technician(s), if available, that allows for maximum physical social distancing. Exterior entrances close to the machine are preferred.
- Provide contact information of person to notify if the above items are not followed (see below)
- Notify Technician(s) immediately if any employee tests positive for COVID 14 days prior to visit or 14 days after.
